Enrolling in Crosby Middle School:
To enroll your child, the online registration application must be completed prior to arriving at the school with the required documents.
A child must be enrolled by the child's parent, guardian, or another person with legal control under a court order. If you are not the parent or legal guardian of a child seeking enrollment, please call the Registrar's Office for more information.
Required Documents for Registration:
The following documents are required in order to complete the registration process:
- Certified Birth Certificate (no copies)
- Social Security Card
- Shot records
- Withdrawal paperwork from previous school or final report card
- Proof of residence (gas, electric, water, or cable bill with current address and parent/guardian name. Must be dated within 60 days)
- Driver's license or official picture ID of parent/guardian
