Crosby, TX (April 1, 2024) - Attention Crosby ISD Parents/Guardians, there are opportunities coming up in May for free heart screenings and $10 physicals for students for the 2024-2025 School Year.
FREE HEART SCREENINGS
There will be free heart screenings on Thursday, May 9 for any student who needs one. These screenings are free of charge thanks to the Cody Stephens Foundation. The student's guardian must complete the permission form. The heart screens are sent to a cardiologist, and the guardians will receive an email from the CHS trainers if the student's heart screen is normal or if a follow-up with a cardiologist is needed.
$10 PHYSICALS
Physicals for the 2024-2025 school year will be available on Saturday, May 18, at the Crosby High School gym. The cost is $10. Use cash, QR code below or this link to pay: https://crosbyisd.revtrak.net/Athletics/#/v/athletics-2024-annual-physical. The QR Code and link to pay won't open until May 15.
Here is a quick explanation of when physicals and heart screenings are required:
Requirements for Physicals/Heart Screenings (7th – 12th grade)
- Athletics/Cheer/Dance:
- Physical/medical history – every year
- Heart screenings – incoming 7th and 11th graders as well as any student new to the district
- Band:
- Physical – 9th and 11th grade
- Medical history – every year
- Heart screening – incoming 9th and 11th graders as well as any student new to the district.
- JROTC:
- Physical/medical history – every year
- Heart screening – incoming 9th and 11th graders as well as any student new to the district.
